Betty H. Matsushita (11 December 1931 – 13 May 2004; age 72) appeared as a hologram of T'Pau in the Star Trek: Voyager episode "Darkling". She did not receive credit for this appearance.
In January 2004, Matsushita was a hospice patient in San Antonia, California. [1] She passed away four months later.
