Surgeon's log, USS Exeter

The Surgeon's log on the USS Exeter was the method used by the chief medical officer to officially record his final log entry. An entry left Commander Carter was discovered in the log tapes by the crew of the USS Enterprise in 2268. (TOS: "The Omega Glory")
